PHS Human Subjects and Clinical Trials Document Templates

All NIH grant applications that involve human subjects are required to complete the Human Subjects System (HSS) in the Application Submission System & Interface Tracking (ASSIST) program. These templates correspond to sections of the ASSIST program which are completed depending on the study type.  They are provided to assist in preparing each document.
